Stock Transfer Information
In order to effect a transfer to a new registration, stock certificates and related documentation should be sent by certified or registered mail to:

The signatures of all owners shown on the face of the stock certificate must be MEDALLION GUARANTEED. A Medallion Signature Guarantee must be done by a brokerage firm or a financial institution that is a member of an approved medallion program pursuant to Securities and Exchange Commission Rule 17 Ad-15.
- Instructions for the new registration should include: the name, address and Social Security number for the new owner.
- Minor accounts must include the name of one adult custodian.
- Trust accounts should include the names of the trustees, the date of the trust, the name of the trust and a Social Security or taxpayer identification number.
